Additional Forms

• If any changes occur, you need to turn in a change form. Changes include: changes in income, additional children, change of marital/domestic partner status, change of contact information, or anything else relevant.
• If you are in a domestic partnership, or if you and your spouse have different last names, you need to turn in a domestic equivalency form, along with appropriate documentation (examples are listed on the form).
• If for some reason you need to file an exception for consideration by the Task Force (for example, if your application was denied), please turn in an exception form.
