O'Brian's Test

 

 

 

Patient Position

The patient should be either sitting or standing.

The Glenohumeral joint should be flexed at 90° and adducted at 15° out in front of the patient.

The patient should begin in complete internal rotation for the first part of the examination with the palm of the hand facing away, and complete external rotation for the second portion, with the palm facing inward.

Examiner Position

The examiner should be by  the patient with one hand on top of the distal forearm .  This should prepare the subject to resist a downward force.

Evaluation

The examiner pushes in a downward force with the patient attempting to resist the force applied. (for both internal and external rotation)

Positive Test Indication

Pain is experienced by the patient in this examination, more in internal rotation than external rotation.

  • AC joint pain may demonstrate AC pathology
  • A labral tear may be present if there is clicking or pain within the Glenohumeral joint
