payroll: Updated Forms for International Employees

The Banner Guide has been updated to include the latest versions of international employee forms; individual forms and bundled hire packets at http://bg.uoregon.edu/content/hris-forms#hris-Forms
 
You may start using the newly published forms for all hires. You need to refresh your browser to receive the latest version of the forms.
 
Please recycle OLD forms and use the latest version of the forms:

• CO-NRA (Foreign National Data Request Form) with 2012 calendar year
• IRS Form 8233 with 2012 calendar year
• No changes to the Attachment Letters for the Form 8233

Note:  Any OLD forms submitted to the Payroll Office may delay in processing the employee's pay.
