Dear Fellow members of the COI/COC Senate Ad Hoc Committee: As promised, I am attaching to this email a document entitled "COI-lite-v2.pdf". In a word this is an attempt to be "everything you ever wanted in a Conflict Of Interest Disclosure Form and Less!". I tried to be minimalist and present a form that is as easy to fill out as possible and is the least intrusive whilst still meeting our legal requirements in this area. I am responsible for this document. However, I had some help in the drafting and in particular "legal" has looked at it and agrees it does fulfil our responsibilities in this area. I emphasize. This comes from me personally. I take sole responsibility for the draft. But I should be grateful if you could take a look at it in advance of Friday's meeting and perhaps we could discuss

• a) Is it in fact easier and less intrusive?
• b) Is it worth considering further or do we toss it in the bit bucket?
• c) What is the next step?
