I still think that it is not necessary to ask EVERY faculty member to fill a form of COI/CD. However, if you allow me, I would like to make some comments/questions on your draft form. For Question 3, should I assume that the usual NSF grant for math is in the intended "yes" category? I believe that when we apply NSF grant, we already disclose this, and the university controls the fund anyway. My question here also applies to the Question 1. If we have already disclosed that we are PI's of NSF grants and related information, how important is it to be asked again? Perhaps the question could be directed to something which has not been disclosed? For Question 4, perhaps a LOWER BOUND should be given here? Otherwise, a person who fills the form would be bothered by some trivialities. For example, somebody was paid 20 dollars for taking care of neighbors pets. It is definitely a non-University professional income-producing activity. But I doubt that anybody (IRA?) would be interested in such items. Back to Question 1, many teaching/research faculty visit other institutes during summer to continue their teaching/research related work. They may receive some form of compensation for these activities. How different is it from the "honoraria" that is mentioned there, in particular, when it is not that significant? (Note also, many of them do not have summer salary).
