PRELIMINARY
Signs and Symptoms: Brain tumor.

MRI OF THE BRAIN WITHOUT CONTRAST

IMPRESSION:
1. The pontine mass appears relatively stable in size, however there has been slight iterval increase in the enhancement portion in the left side of the mass.
2. Stable ventricular size without evidence of hydrocephalus.


HISTORY: Four year old female with brain tumor.

TECHNIQUE: Multi-sequence, multi-planar MR images are obtained of the brain prior to and following intravenous administration of 3.5 cc of Magnevist contrast. The patient was sedated during the procedure with 70 mg of Nembutal.

COMPARISON: Prior study from 9/15/05.

FINDINGS: Ventricular size is stable. There is no evidence of hydrocephalus.

The pons is expanded by an infiltrating mass. It is predominantly hyperintense in the T2 and FLAIR sequences. It is similar in size to the prior study, measuring approximately 4.8 x 3.4 x 3.6 cm in transverse, AP and craniocaudal dimensions respectively. The mass extends into the prepontine cistern and infiltrates into the left cerebellar peduncle. Following gadolinium administration there is an enhancing portion in the left lateral aspect of the mass. This has increased slightly in size since the prior study. The enhancing portion measures approximately 19.2 x 14.5 mm (previously 14.4 x 14.8 mm at a similar level).

The vascular artery is anteriorly displaced and the mass envelopes the artery nearly completely. This appearance is similar to the prior study.

There are flow voids in all of the major vascular structures, indicating patency.

Mild mucosal thickening is seen in the maxillary sinuses.

Mastoid air cells appear clear.

Orbits appear symmetrical and mormal.
