Reason for Exam: Follow up on brain tumor.


MRI BRAIN - WITH AND WITHOUT CONTRAST

IMPRESSION:
1. Heterogeneous infiltrating lesion centered on theleft pons is again identified. There is decreased enhancement in T2 abnormality at the central portion of the lesion suggesting favorable response to treatment.
2. There is interval development of a new subependymal nodule in the left frontal lobe suggesting progression of tumor. The right frontal lobe subependymal nodule has also increased in size in the interval.
3. Stable appearance to the ventricular system.


INDICATION: Brain tumor.

TECHNIQUE: Multiplanar MR imaging of the brain was performend with and without contrast using the pediatric protocol 4 cc of Magnevist.

COMPARISON: Prior MRIs dated 7/10/06, 4/14/06 and 9/15/05

FINDINGS: The infiltrating neoplasm involving the left pons is again identified. There is expansion of the pons with mass effect on the 4th ventricle. There is marked decrease in the enhancement on the current exam. There is heterogeneous signal intensity within the left aspect of the pons consistent with sequelae of hemorrhage and/or calcification. There is increased T2 signal intensity within the left pons extending into the right pons consistent with vasogenic edema and/or tumor.

A ventricular shunt from a right frontal approach is again identified. The ventricular system is stable in size. Again identified is a subependymal area of increased T2 signal intensity in the right frontal lobe. On the current study, there is a new lesion identified in the subependymal region of the left frontal lobe. Findings are worrisome for a subependymal spread of tumor.

No significant extra-axial fluid is seen. There is crowding of the basilar cisterns. The central flow voids are maintained. The paranasal sinuses are well aerated.
