payroll: Employee Information Form Instructions
At the recent HRIS Meeting, a new HRIS form was launched. The Employee
Information Form (EIF) was named by attendees at the meeting. As stated
in the September HRIS Notes, this form replaces the PIF, UO ID Request
Form and both NEI Forms. We are still fine-tuning the process, and are
offering number of options for the manner in which departments obtain
Section 1 information.
The EIF and Instructions are now located on the forms page of the Banner
Guide.

Note: Forms are located in three different sections on the page: Hire
Documents, Individual Listing, and Bundled Packet Sections.
Important points:
EIF
- The signature of the employee may be obtained in Step 1 or later
in Step 4.
- Photo ID requires that the UO ID be written on the EIF.
UAF
- The Unpaid appointment Form (UAF) should be used when sending
individuals on courtesy, emeritus, retired, post doctoral, and associate
appointments to Photo ID.
- Photo ID requires that the UO ID be written on both the UAF form.
- We will be revising the Unpaid Appointment Form very soon. For
the time being, please indicate on the current UAF if the individual will
need to be issued a Proximity Card.
- We are changing our procedure: Upon receipt of the UAF, payroll
staff will notify departments of the newly created UO ID number. For
recently submitted UAFs, check Banner if you don't know the UO ID Number.
There will be an on-going effort to update the Banner Guide, removing
references to the PIF, UO ID Request Form, and NEI forms. This may take
awhile.
Please contact me or others in the Payroll Office with your comments on
the form itself or issues you may be experiencing with the process. We
will attempt to address all issues by PAYNEWS, and will not be scheduling
a second meeting this month for those who were unable to attend last
week's meeting.
