This undated print is a woodblock reproduction of a 1920 design by the artist created for the music publisher Senoo Ongaku Shuppansha.  The song title, in the left column in the upper right of the print, transcribes as:

花とならばや
Hana to naraba ya

The caption モツアル卜作曲 below the catalog raisonne entry below states the lyrics have been set to a composition by Mozart. The line above reading 惟一くらぶ作歌 , seems to indicate the work was created for a song or poetry club.  The catalog raissonee indicates the song sheet was the 15th in a series of 35 ballads published by this club.
