We
have changed the Airfare Authorization form slightly and added a new field.
There is a new field under the travelers name which says UO ID#.
Please enter the travelers 95X number or V number. If the traveler does
not have an ID number please leave the field blank. The new field allows
us to image the document saving time and space. If you have this
form saved on your computer, please delete it and use the new form. You
